Dr. Cantley evaluates patients on a private pay basis. She does not contract or participate with any health insurers.
Fees vary depending on the type of evaluation, but in accordance with The No Surprises Act as outlined in Federal & State law, good faith estimates are provided on an individual basis.
Private pay patients must agree to and sign The No Surprises Act Standard Notice & Consent Documents in order to receive services.
Patients seeking out-of-network care may seek reimbursement from their carrier directly.
All forensic evaluations are private pay. Services are not considered medically necessary, and therefore, are not billable to insurance, even as out-of-network.
Payment is expected in full at time of service(s).
Methods of payment include check, cash, Visa, MC, Discover, AMEX, and HSA cards (with any of these logos).
Patients are fully responsible for researching their insurance benefits and filing all claims, if applicable.
